A 63-year-old patient presented with cough and pleural effusion. Pleural cytology was positive for adenocarcinoma of unknown primary, while bronchoscopy was negative and CT scan, inconclusive. Sestamibi scintigraphy showed a thoracic mass with a necrotic centre in the right lower lung suggestive of lung cancer [
