Restricted diffusion in the brain on MRI: do not stop at stroke

Imaging findings in early cerebritis resemble that of acute ischemic infarction on diffusion weighted MR images. We report a case of early cerebritis which mimicked acute infarction on initial presentation and progressed rapidly into a full blown cerebral abscess requiring surgical intervention. To the best of our knowledge only one prior report on misinterpretation of cerebral infection as infarction on imaging, more specifically on CT, exists in the literature. The authors of that report did not attempt to differentiate the two etiologies, however [1]. …
