A 50-year-old male, known to have diabetes mellitus, presented to the emergency department with acute dyspnea. His past medical history was notable for symptoms suggestive of deep vein thrombosis in his right lower leg one month prior (the patient did not seek medical evaluation at that time). He was diagnosed with pulmonary embolism in our service, likely secondary to untreated deep vein thrombosis. Transthoracic echocardiography detected right ventricular systolic dysfunction and a patent foramen ovale with a floating thrombus through the interatrial defect between the right and left atrium. …
