Shiota et al. investigated the risk of recurrence in lymph node-positive prostate cancer patients without prostate-specific antigen (PSA) persistence.1 They developed a multi-institutional prognostic model to guide treatment decisions. Several prognostic factors associated with recurrence-free survival were identified, including elevated PSA levels, a high International Society of Urological Pathology grade group based on radical prostatectomy specimens, pathological T stage, and positive surgical margin status. The authors emphasized that their model, incorporating postoperative PSA and the number of involved lymph nodes, has high clinical utility for guiding patient discussions regarding treatment options. …
