Prognostic relevant factors within pT3 rectal carcinomas: depth of invasion or circumferrential resection margins?

The hallmark of optimal rectal cancer surgery is a perfect mesorectal excision 5 cm beyond the caudal tumor edge. This technique has been a necessary part of standard rectal carcinoma surgery for more or less 10 years in most places. However, it is not clear in which patients undergoing mesorectal excision radiotherapy in addition to surgery is able to reduce the frequency of local recurrence and should be applied pre- or postoperatively. This is particularly true for pT3 rectal carcinomas which exhibit a pronounced prognostic inhomogeneity. …
