Sonographic B-line, which indicates contact of visceral pleura with parietal pleura, is highly sensitive finding, which enables ruling out pneumothorax. The images herein were obtained from an 80-year-old man with refractory pneumothorax on the right side. On lung ultrasound, the right anterolateral chest wall showed A-lines without lung sliding and stratosphere sign in M-mode. Furthermore, comet-tail artifacts, mimicking B-lines were noted in the region where the visceral pleura did not contact the parietal pleura (Fig. 1; Video 1).
