A 68-year-old female with a background history of hypertension and idiopathic peripheral neuropathy presented with a 24-h history of increasingly severe abdominal pain, diarrhoea, nausea and vomiting. On examination, she had evidence of peritonism in the left iliac fossa. Blood results demonstrated an inflammatory response with a raised white cell count (14.1 × 109 cells/l) and raised C-reactive protein (64 mg/l). Abdominal CT imaging revealed a likely perforation at the distal sigmoid and the patient proceeded to laparotomy. …
