A 69-year-old woman was diagnosed in 2013 with 20 mm in diameter pancreatic cyst that on MRI revealed a connection with the main pancreatic duct (MPD) (Fig. 1a). This was considered a presumed Branch-duct intraductal papillary mucinous neoplasm (BD-IPMN) and no other worrisome features or high-risk stigmata were present; thus, she was enrolled in a surveillance program.
