Osteoarticular tuberculosis: in search of new biomarkers

Abstract

Background

Our objectives were to evaluate the diagnostic utility of serum and synovial fluid concentration of adenosine deaminase, interlukin-6, tumor necrosis factor-alpha, and interferon-γ in patients of osteoarticular tuberculosis (TB) and define a cutoff limit of these markers in the serum and local fluid for its diagnosis.

Methods

One hundred twenty-two serum and local fluid samples collected from clinically suspected patients of osteoarticular TB were screened for adenosine deaminase (ADA) and cytokines along with microscopy, culture, and histopathological examination.

Results

After exclusion of 39 patients, in whom exact diagnosis could not be confirmed, remaining 83 patients were divided into two groups, TB (n = 53) and non-TB (n = 30). We found a statistically significant difference in the levels of serum and fluid ADA, and also in serum levels of TNF-α, between the two groups. The difference in the ratio of serum (tumor necrosis factor (TNF)/IL-6) and fluid (ADA/IFN-γ) was also found to be statistically significant.

Conclusion

Local fluid ADA, along with serum ADA, and TNF-α levels can be useful biochemical markers for the diagnosis of osteoarticular TB.
