Periumbilical swelling, erythema, and discharge in a girl: Answers

The imaging undertaken to understand the reason for abdominal distention, seropurulent discharge in umbilicus, ulcer, and constant high fever showed the contrast material given during voiding cystourethrography (VCUG) leaking into the retroperitoneal area. Abdominal computed tomography (CT) revealed more apparent subcutaneous inflammation on the left and remnant of the urachal canal. The case was diagnosed as an infected remnant of the urachal canal and secondary retroperitoneal bladder rupture.

The urachal canal remnant was resected due to subcutaneous inflammation developed secondarily to the infected urachal canal remnant and bladder rupture. A vesicostomy was opened to protect the upper urinary tract from experiencing high intravesical pressure and recurrent febrile urinary tract infections (UTIs). These surgical procedures were completed without complications and following completion of antibiotic treatment, the case was discharged to come for follow-up.
