MRI at presentation shows focal decreased signal intensity on T1-weighted imaging (Fig. 1a) with corresponding increased signal intensity on the water sensitive sequences (Fig. 1b and c), consistent with oedema-like changes in the bone marrow within the spinous process of C7 (white arrows). CT (Fig. 2) was subsequently performed and demonstrates a focal expansile lucency at the tip of the C7 spinous process with focal cortical destruction (white arrow).
