Immediately after stabilization of the patient’s condition, all persons involved were debriefed, and all actions preceding the event were painstakingly revised. All solutions, materials and disposables used in the procedure were saved. The clocks of the various instruments were “synchronized” to a single accurate clock time, and all monitor recordings were saved. …
