We read with great interest the letter from Dr. K. H. in’t Hof and his colleagues concerning our article entitled Minilaparoscopic Appendectomy Using a Needle Loop Retractor Offers Optimal Cosmetic Results (Surg Endosc 18: 1578–1581, 2004). The one-trocar technique they mention in the letter is an excellent technique that has been attracting much attention in the field of pediatric surgery. Although its indication is limited by the position of the vermiform appendix or adhesions, we believe that it may be superior to our technique in terms of the cost–benefit ratio because the approach to the mesoappendix and appendiceal root is similar to that in open surgery. …
