Cancer in the thyroid isthmus raises concerns about extrathyroidal extension (ETE) because of its thinness.1 Previous studies have shown that papillary thyroid carcinoma (PTC) of isthmus origin is more likely to cause ETE compared with those of lobe origin. However, there are currently no existing guidelines outlining the surgical scope of isthmic PTCs.2,3 Given the controversy over surgical extent for isthmic PTCs, the isthmic PTC requires focused attention regarding its clinicopathological behavior. …
