With the increasing need for surgery in elderly lung cancer populations, adequate preoperative evaluation is of paramount importance.1 Frailty is an emerging concept in recent years, and the predictive value of the associated 5-factor modified frailty index (mFI-5) has been well demonstrated in preoperative evaluation in most surgical areas,2 but a gap remains in thoracic lung cancer surgery.3
