ASO Author Reflections: Is Breast-Conserving Surgery a Safe Treatment Option for High-Risk Breast Cancer?

After the identification of BRCA genes in the early 1990s, scientific research has ascertained the enhanced inherited susceptibility for breast cancer (BC), intrinsically linked to mutations in these genes. Indeed, an elevated risk of both local recurrence, ipsilateral to the index tumor, and subsequent contralateral cancer has been recognized in BC patients carrying germline high-risk pathogenic variant mutations.1 Furthermore, a very high risk of development of a first new BC diagnosis in asymptomatic individuals has been reported.2 Surgical preventive managing of such typology of women has evolved accordingly, based on international scientific guidelines,2 consolidating the predominant role of bilateral mastectomy, very frequently used and considered the only life-saving option. …
