ASO Author Reflections: Lymphadenectomy in Esophagectomy: Why Bother?

There has been much debate regarding the necessity of lymphadenectomy in treating esophageal cancer. While some acknowledge its importance in improved staging, others have argued that it conveys no curative benefit. Neoadjuvant therapy for patients with locally advanced disease is recognized as standard practice to address any potential disease spread.1 However, the extent of lymphadenectomy has previously been conflated with the number of nodes removed, and the two are not the same. Previous studies have shown benefit for a thorough lymphadenectomy in patients receiving neoadjuvant treatment.2,3 What of the patients who have early disease and do not receive this, or where neoadjuvant treatment is contraindicated? …
