Value of Omentoplasty in Abdominoperineal Resection with Primary Perineal Closure for Rectal Cancer

We read with immense interest the article titled, “Snapshot Study on the Value of Omentoplasty in Abdominoperineal Resection with Primary Perineal Closure for Rectal Cancer,” published in March 2018 by Blok et al.1 A cohort of patients was assembled from all patients undergoing APR for rectal cancer at 71 Dutch centers in 2011. A total of 477 patients met the inclusion criteria of nonlocally advanced tumors and primary perineal closure, and 36% (172) underwent omentoplasty. There was no significant impact of omentoplasty on the nonhealing of perineal wounds at 30 days or at the end of follow-up, pelvic abscess formation, or reintervention for ileus. Moreover, perineal hernia appeared to be more common after omentoplasty (13% vs. 7%), and this difference persisted after conducting a multivariable analysis (odds ratio [OR] 2.61, 95% confidence interval [CI] 1.271–5364, p =0.009). The authors concluded that omentoplasty after APR with primary perineal closure does not improve perineal wound healing and appeared to increase the occurrence of perineal hernia. …
