Regional lymph node status (N status) is a reliable prognostic indicator for patients with radically resected gastric cancer. The simple and well-established number-based AJCC pN classification has been widely used for clinical prognostic assessment of N status for decades. However, 2 ratio-based classifications, metastatic lymph node ratio (MLR) and log odds of positive lymph nodes (LODDS), have been proposed and were recently reported to be better than pN in predicting accuracy.1 , 2 Controversy over how to choose the most appropriate N classification still exists. Some researchers suggest assessing the prognostic value according to the hazard ratio (HR) of each N classification.3
