Selection of patients with advanced disease who should undergo aggressive therapy remains controversial. Analysis of outcomes following treatment of appropriately selected patients with advanced HCC, specifically those with large or multiple tumors or major vascular invasion, reveal that significant progress has been achieved in selection for surgery and the indication for systemic chemotherapy and arterial chemoembolization. This article examines current approaches for the surgical treatment of large HCC, multinodular HCC, HCC associated with major vascular invasion, and examines combined approaches for the treatment of advanced HCC. …
