Clinical manifestation of uremic encephalopathy is variable including altered consciousness, confusion, seizure, or movement disorders such as tremor, myoclonus, dyskinesia, or Parkinsonism. Typical radiologic manifestation of uremic encephalopathy is involvement of basal ganglia, cortex or white matter, except thalamus [

2]. Recently, Jia et al. reported a case of uremic encephalopathy with metabolic acidosis showing isolated brainstem lesion [
