Parathyroid adenoma is the most frequent cause of primary hyperparathyroidism, followed by hyperplasia. Parathyroid adenoma can be seen in various locations including ectopic locations like the thymus, carotid sheath, soft palate, and mediastinum. It is reported that the incidence of the ectopic parathyroid gland is around 6–16% in adults. These ectopic parathyroid adenomas are the major causes of failed primary parathyroid surgery. Here, we are presenting a case of ectopic parathyroid adenoma in the carotid sheath which was managed and cured in the initial surgery.
