Situs ambiguous or heterotaxia is a rare condition, with an incidence of 4 per 1 million live births [1], which occurs due to a disruption during the embryonic development of the orientation of the axis, which can lead to multiple alterations in the presence, location, and function of thoracoabdominal organs. Most affected persons die by the age of 5 years, mainly because of cardiac anomalies. Situs ambiguous is traditionally divided into two general categories: left isomerism (situs ambiguous with polysplenia) and right isomerism (situs ambiguous with asplenia) [2‐4]. It is recommended that several critical structures be evaluated by the radiologist: position of the cardiac apex and atria, position of aorta and venous drainage below the diaphragm relative to midline, position of the stomach and presence or absence of malrotation, position of the liver and gallbladder, presence and number of spleens, and presence of tri- or bilobed lungs. …
