Surgical dogma has historically mandated operative management of small bowel obstruction (SBO) in patients with no previous history of abdominal operations, due to potential malignancy as the underlying etiology [1]. However, recent data suggest that adhesive disease is the leading cause of SBO in these patients, similar to those with a surgical history [2, 3]. The rates of newly diagnosed malignancy in SBO in the virgin abdomen also appear to be low, ranging from 3 to 13% [26]; hence, the management is evolving. …
