We would like to thank Dr. Nanavati for his critical review of our manuscript and for posing such insightful and thought-provoking questions. Despite knowledge of the association between blood transfusion and immunosuppression over the last 40 years, relatively little progress has been made in elucidating the specific mechanism(s) underlying this phenomenon, especially with regard to immunodysregulation preceding cancer recurrence. Though many theories have been postulated, including downregulation of the body’s endogenous immunosurveillance cells, they remain just that: theories. This magic bullet currently remains unidentified; more importantly, the means to stop it remain elusive. …
