Concerns about the long-term safety of live kidney donors are justified

Healthy live kidney donors lose approximately 50% of their nephron mass with nephrectomy [1]. Within the first post-donation year, the remaining kidney partially compensates for this loss by means of hyperfiltration, a homeostatic mechanism by which the remnant functional units of the kidney increase in volume and capacity in an attempt to match demand [2]. For this reason, the residual kidney function (glomerular filtration rate, GFR) is typically only 20–45% lower than it was before nephrectomy [3]. …
