The technique of transanal total mesorectal excision (taTME) using the transanal minimally invasive surgery (TAMIS) platform [1, 2] remains in the early phase of adoption [3], and important changes have been made which are part of the natural evolution process. Despite an enriched experience, we are collectively still in the 2b-‘Exploration’ step of the IDEAL framework described by McCulloch et al.
