Primary inguinal hernia repair: open or laparoscopic, that is the question

It is perhaps a sign of senescence, if not obsolescence, to be asked to write on the subject of open inguinal hernia repair in a journal devoted to minimally invasive surgery. Nevertheless, there remain some significant reasons to consider open inguinal herniorrhaphy a preferable approach in the management of groin defects. Some of those features will be herein reviewed. This will not be approached as an exhaustive, scholarly effort, but rather as a position paper, with arguments derived from the author’s experience, as well as the available literature. As a practitioner who performs both open and laparoscopic hernia repair in practice, what is presented outlines why the author chooses to perform most inguinal hernia repairs via the open technique. …
