Ischaemic stroke of the artery of Percheron (2007: 10b)

A 62 year old male patient was admitted into hospital one day after an acute onser of headache, rapidly followed by visual disorders, inability to walk and unconciousness. In the patient's past history there were episodes of intermittent atrial fibrillation. Therefore the patient had been treated with long term anticoagulants. However, the therapy had been discontinued by the patient. On admission cranial CT (Fig. 1) was performed along with CT angiography. At this stage the patient was comatose. MR was also performed (Figs. 2 and 3). A CT-angiogram presented a patent basilar artery. As a normal anatomic variant there was no P1 segment on the left with the posterior cerebral artery arising from the internal carotid artery. There was no relevant stenosis. The major venous structures were also patent. There was no haemorrhage or signs of increased intracranial pressure. At this stage the patient was comatose. …
