Surgery is a complicated science dealing with intervention modalities to treat patients, and there is much more to it than just cutting and suturing. The surgical literature, accordingly, has become more sophisticated, with a new concentration on trials and favorable statistics, showing surgical practice to be more and more evidence-based (a term that should be mentioned at least once in any discussion of modern surgical practice). At the same time, one should never forget that surgical technique is still the essence of our profession, and without it we cannot be called surgeons. Also, good surgical technique, with the right instruments, allowing us to complete the job safely and effectively, is still the key to good patient outcome. Nevertheless, one should never forget that modern surgery as practiced in centers of excellence and described in surgical journals and conferences is not within the reach of most of the world’s population. While famine is still prevalent, expensive surgical technology is something most patients, and surgeons, around the globe can only dream of. …
