A 27-year-old woman was referred for further evaluation of a fetal lung lesion identified on routine prenatal US. MRI at 25 weeks’ gestation showed an enlarged, but structurally intact, left upper lobe with stretched hilar vessels causing mediastinal shift to the right and compression of the left lower lobe posteroinferiorly (not shown) consistent with congenital lobar emphysema (CLE) (Figs. 1 and 2). Follow-up US demonstrated partial regression and slightly decreased echogenicity of the lesion. The diagnosis was confirmed at surgery after birth, undertaken because of respiratory distress.×
