Sir: The esthetic quality of a postmastectomy breast reconstruction is closely linked with the definition of an inframammary fold symmetric to the contralateral one. The new techniques of skin-sparing mastectomy, preserving the skin and the fascial system of the natural inframammary fold, have certainly reduced the number of reconstructions in which the surgical creation of a neofold is necessary. In many cases, however, it is still necessary to restore the anatomical definition of this particular aspect of the reconstructed breast, mainly in cases of prosthetic reconstruction. As is known, an anatomically correct inframammary fold is marked by a correct breast thoracic wall angle for the lateral view and a semicircular arch from the frontal view [
