Clinical vignette A previously healthy 51-year-old woman was admitted to the intensive care unit (ICU) with severe community-acquired pneumonia. She required intubation and mechanical ventilation 6 h after admission. Her respiratory status declined continuously over the next few hours, and her PaO2/FiO2 ratio was 65 with a positive end-expiratory pressure (PEEP) at 8–12 h after admission (plateau pressure was 28). She was hemodynamically stable and had normal renal function. …
