Freiburg Neuropathology Case Conference: Contrast-enhancing White Matter Lesion Adjacent to the Lateral Ventricle

A 48-year-old right handed woman presented with progressive left side hemiparesis for over 2 weeks. Initially, she observed a mild gait disturbance with a weakness of her left leg, followed by a paresis of her left arm and a hemihypoesthesia of her left side. Nine years earlier the patient was diagnosed with psoriasis. As a result of the discussion of this case in our interdisciplinary board, the recommendation for stereotactic serial biopsy was given to ascertain the histopathological diagnosis. The biopsy was performed as a minimal-invasive stereotactic procedure under local anaesthesia using a stereotactic frame (Riechert–Mundinger, MHT, Bad Krozingen, Germany). Under the perioperative therapy with dexamethasone an improvement of all symptoms was observed. …
