Performance of the Zweymüller total hip arthroplasty system: a literature review including arthroplasty register data

Abstract

Materials and methods

A structured literature review was performed using a standardised methodology to determine the outcome of Zweymüller implants, check the quality of the basic data and identify potential bias factors. Apart from a comprehensive meta-analysis of clinical literature, worldwide register results were also taken into account. The main indicator was the revision rate.

Results

Overall, the comparison yielded good results for Zweymüller hip arthroplasties. However, the datasets from clinical studies show a wide range of variation, particularly as regards follow-up examinations over longer periods of time. The comparative evaluations have not revealed any inexplicable deviations or bias factors in the datasets, except for the Bicon and SL-Plus implant combination, which exhibits a relevant proportion of publications dealing with the problems of a historical low-carbide metal–metal pairing. The conclusions drawn are not uniform. Scientific publications do not provide a sufficient basis to retrospectively deduce the actual problem or the decision-making process.

Discussion

For the Zweymüller prosthesis, a sufficient number of publications are available that allow for robust statements regarding the outcome also in comparison with other implant systems. The results of this implant can be rated as good. Nevertheless, the data review has shown several weaknesses in the details that can be considerably reduced through the use of register results. Employed as a benchmark, register data are able to provide valuable additional information for the assessment of clinical studies.
