Comparison of Transurethral Surgical Methods

Abstract

Transurethral surgical techniques are an integral part of the urologists’ armamentarium for the treatment of lower urinary tract symptoms caused by benign prostate enlargement. Besides monopolar transurethral resection of the prostate (M-TURP), which can be regarded as the gold standard and reference technique, several established surgical alternatives are available. Bipolar TURP offers comparable functional outcomes to M-TURP at a lower rate of perioperative morbidity. Modifications of the bipolar technique such as vaporization or enucleation show promising results but need further investigation. Fewer complications and comparable long-term outcomes are found after holmium laser enucleation of the prostate (HoLEP) when compared with M-TURP. GreenLight laser vaporization (GLV) of the prostate is also associated with fewer perioperative complications than TURP and comparable short-term outcomes. Thulium laser enucleation of the prostate resembles HoLEP but still lacks long-term results from prospective randomized trials. The same is true for diode lasers, which need to be further investigated in high-quality studies to acquire sufficient evidence.
