Aberrant Right Hepatic Sectoral Duct Injury Following Laparoscopic Cholecystectomy: Evaluation and Treatment of a Diagnostic Dilemma

Bile duct injury (BDI) and postoperative biliary leaks are common complications following laparoscopic cholecytectomy. Biliary damage and leaks can often be easily diagnosed by an intraoperative cholangiogram, postoperative endoscopic retrograde cholangiopancreatography (ERCP), or percutaneous transhepatic cholangiogram (PTC). We present a case of a persistent biliary leak with biloma associated with unrevealing intraoperative cholangiograms, ERCPs, and PTCs. The final diagnosis of an aberrant right hepatic sectoral duct injury with a posterior sector bile leak was suspected and ultimately confirmed during a repeat operation that was required to repair the persistent biliary leak. …
