Transanal total mesorectal excision (taTME) using the technique of transanal minimally invasive surgery (TAMIS) [1] represents an important development toward improved access to the distal rectum which may improve surgical resection quality [2]. While experience is developing rapidly, the safe implementation of this challenging technique remains crucial [35]. Although uncommon, urethral injury during taTME remains an important, procedure specific morbidity. However, with appropriate training, and a clear understanding of critical anatomic landmarks—which are unique to the taTME perspective—this can be avoided. …
