Endosonographische Darstellung der Tumordicke von oralen Plattenepithelkarzinomen und deren Einfluss auf die Inzidenz von Lymphknotenmetastasen

Zusammenfassung

Hintergrund

Verschiedene Untersuchungen an oralen Plattenepithelkarzinomen konnten nachweisen, dass eine zunehmende Tumordicke mit einer erhöhten Inzidenz von zervikalen Lymphknotenmetastasen und einem verkürzten Überleben assoziiert ist. Bislang wurden Untersuchungen zur Tumordicke anhand von histologischen Schnitten retrospektiv durchgeführt. Das Ziel der vorliegenden prospektiven Untersuchung war es, ein hochauflösendes Ultraschallsystem zur intraoralen Messung der Tumordicke zu evaluieren sowie die prätherapeutisch gemessenen maximalen Tumordicken mit dem N-Stadium und dem Gesamtüberleben zu vergleichen.

Patienten und Methoden

Insgesamt wurden 64 Patienten mit primären Plattenepithelkarzinomen der Mundhöhle (Stadium I–IV) eingeschlossen. Mithilfe einer 7,5-mHz-Sonde (Hitachi, EUB F334) war in allen Fällen die transorale Darstellung der Karzinome als echoarme, irregulär begrenzte Formation möglich.

Ergebnisse

Der Mittelwert der gemessenen Tumordicke betrug 14±7 mm. Die Tumordicke war in der N+-Gruppe mit 15±7 mm (p =0,032; t-Test) größer als in der N0-Gruppe mit 12±6 mm. In der Gruppe der T1/T2-Tumoren betrug die durchschnittliche Tumordicke 10±5 mm im Gegensatz zu 16±6,6 mm bei den T3/T4-Tumoren (p <0,001; t-Test). Patienten mit einer Tumordicke >14 mm wiesen ein kürzeres Überleben auf als Patienten mit Tumoren <14 mm (48,9 vs. 28,3 Monate; p =0,0102; Log-Rank-Test).

Schlussfolgerung

Die Endosonographie eignet sich als einfaches und kostengünstiges Verfahren aufgrund der hohen Ortsauflösung und des guten Weichteilkontrastes für das T-Staging von Mundhöhlentumoren. Insbesondere bei lokal begrenzten Tumoren lassen sich durch die Messung der Tumordicke prognose- und therapierelevante Daten erheben.
