We describe a patient presenting with coronary dilatation close to a stented segment 9 years after previous intervention complicated with extensive dissection. Using intravascular ultrasound (IVUS) guidance, we were able to obtain useful information on the anatomy of the lesion to confirm the presence of a true aneurysm and its relation with the stent and to visualize a coexisting coronary stenosis. Moreover, IVUS imaging proved to be invaluable in guiding stepwise expansion of a further bare-metal stent until full coverage of both lesion and aneurysm was obtained. …
