Surgical guidelines are like a double-edged sword. They assist surgical practice according to standards of care, based on the common wisdom and experience of the “surgical community”—fortified by evidence, when available. However, some surgical “truths” prove to be dogmas that may be challenged, whereas others may be correct for most cases but inappropriate in specific situations. Having a “non-textbook case” seems so common that we often find ourselves wandering whether the guidelines we read are really relevant. …
