We greatly appreciate the interest expressed by Di Saverio and colleagues in our article titled “Male Gender Is a Risk Factor for Recurrent Appendicitis following Nonoperative Treatment” [1]. Recent evidence from randomized controlled trials [2, 3] supports nonoperative treatment for acute appendicitis, but follow-up analyses for recurrence have been lacking. In contrast, our results provide evidence that roughly 20% of adult patients “selected” for nonoperative treatment experience recurrence. Our study did have some limitations, however, including selection bias. …
