PET-CT: a matter of opinion?

In response to the editorial by Ell and Von Schulthess [1], we feel it is important to raise some questions on the PET-CT storm that is sweeping the world. Although the paper is enthusiastic and positive, one vital question was not addressed, namely, How often is image fusion actually needed? Putting it in a somewhat simplified form, image fusion has three levels: (1) visual fusion of separate CT and PET, (2) software fusion and (3) hardware fusion like the combined PET-CT scanners. After establishing the actual necessity of image fusion, the crucial question is then, Which level is needed? It is remarkable that hardly any literature exists on these relevant questions. Currently, the discussion appears to be based solely on "expert opinions". …
