A 4-year-old boy presented with a systolic cardiac murmur. Coronary CT angiography with oblique coronal reformatting showed an outpouching aneurysm at the right sinus of Valsalva (Fig. 1asterisk aneurysm, arrows right coronary artery, arrowheads aortic valve). The 3-D volume-rendered image clearly demonstrates that the aneurysm is free from the right coronary artery. The aneurysm was surgically closed and no recurrence was noted after a 1-year follow-up.
