The high prevalence of an elevated body temperature in critically ill patients (sepsis, systemic inflammation and neurological presentations) contrasts with the reliability of the evidence that is available to inform the clinical question of “should we treat fever?” [1]. Recently available high quality studies have enhanced our understanding of the potential role of fever control in critically ill patients (Fig. 1) and several clinical trials are currently ongoing.
