Optic nerve ultrasound: artifacts and real images

Sir: I read with great interest the papers of Geeraerts et al. [1, 2] on ultrasound evaluation of the optic nerve and its correlations with intracranial pressure. In my opinion, however, what the authors indicate as the optic nerve on ultrasound examination is an artifact (acoustic shadow), probably cast by the lamina cribrosa. The measurements that the authors report do not reflect the optic nerve sheath diameter, but the measurement of an artifactual image. …
