Does immunonutrition in patients with sepsis do more harm than good?

Almost 2 years ago, we published a meta-analysis of immunonutrition that demonstrated that the overall treatment effect associated with these diets in critically ill patients was a risk ratio (RR) of 1.18 (95% confidence intervals, 0.88, 1.58) [
1]. While there was a clear and convincing signal that immunonutrition was beneficial to patients undergoing elective surgery, there was a suggestion that some sub-populations of critically ill patients were being harmed by the intervention, whereas others experienced a beneficial or neutral effect. Upon further examination of individual studies, important clues as to which population was harmed by the intervention became evident. …
