Sir: The practice of administering alkaline buffering solutions in severe metabolic acidosis is common [1]. Sodium bicarbonate (NaHCO3) is frequently used for this purpose, although it has three important disadvantages: initially a further decrease in intracellular pH can occur, CO2 is generated, and it carries a significant sodium load. THAM (tris-hydroxymethyl aminomethane, or tromethamol/TRIS) is an alkaline buffer without these disadvantages. THAM's advantages have recently been stressed, although no controlled studies are available [2, 3, 4]. …
