To the Editor: We have read with great interest the article “Endoscopic management of pineal cyst-associated aqueductal stenosis” by Laurence Davidson [
1]. In this article, the author concluded that endoscopic surgery, including endoscopic third ventriculostomy (ETV), is effective for symptomatic pineal cyst (PC). Although the surgical strategy for symptomatic PC, which is an extremely rare entity, is controversial, total resection of this benign cyst in craniotomy is not mandatory. Therefore, with the recent development of endoscopic neurosurgery, it makes sense that less-invasive endoscopic surgery should be prioritized over microscopic neurosurgery, as reported by the author [
