The authors congratulate Dr. Yamashita and colleagues on their study, which demonstrates that using a preoperative Comprehensive Geriatric Assessment (CGA) to assess frailty in older persons can identify patients at increased risk of specific complications after esophagectomy.1 With an increasing number of elderly patients undergoing curative surgery for esophageal cancer, it is important to explore the role of frailty assessment in predicting surgical outcomes to better support decision making for these patients.2 We appreciate the authors detailed and careful analysis on the role of frailty indices and reflect on its applicability to clinical practice in our local context. However, we wish to highlight some important limitations in the use of the CGA as described in this study and welcome the authors views on suggestions for additional areas for analysis within the cohort. …
